Procedure for applying for a new connection

HiSo I got this form from MTNL with like 25 fields and what I need to know is that will this one form cover everything I want? Firstly I want to apply for TriB UL 512 (1M/1M) and I also would be requiring a new telephone connection. So would one form suffice for both? Also is there going to be another plan for the telephone bill or what? Would the 999 pm cover the telephone or would I need to pay for the telephone as well?Thanks

I am also getting this plan, i called 22221500, they told me there is no charge for phone in Rs999 plan.

Last year I paid Rs 500 only as installation fees. Phone instrument was provided free of cost by MTNL and I bought my own modem from the market. I opted for 256 kbps unlimited plan for Rs 599. The plan was known as Trib 256 UL combo. The bill I get is for Rs 599 plus service tax only. This include 100 calls on the phone for free to any network.Just submit the form with an ID/address proof to MTNL with fees in cash. New connections are installed in 2-3 days in most areas.
