I have forgot my Portal ID and Password

Hi I have forgot my BSNL Broadband Portal ID and password. how could i get it generated? i want to check account usage also. help me. I have downloaded Data One Quick Usage Finder but that is also now working . error is saying that "could not access Data One site plz ensure that u r connected".
