Do I will have to return both the telephone instrument and modem or I could just return the modem

Can I surrender the modem at any sanchaar hat or at my exchange too ?

What documents to carry to get in done in a single day ?

You have to go to your exchange to surrender your modem.
you need to write an application and get it xeroxed ( 3 copies are needed)
If you have taken the telephone then you have to return that too.
