I recently (read a couple of months back) ported my prepaid SIM. I had to submit and verify my documents before getting it activated.
@meetdilip had you changed ownership while porting ? according to guidelines of TRAI one has to undergo verification in order to activate sim either new connection or port-in

Nope. Ownership remains the same. Even with that they asked for verification documents.

You can change ownership via MNP. Simply apply at your new operator with requisite proofs. Proofs don't need to be on the name of the previous owner.
