use the first form, there is a option in the end 'Other Violation (describe below)' tell google how the pub id ad code is being injected to webpages via javascript

also the ads appears on all non https site that includes p0rnographic too which is serious violation

For Google adsense you need at least one working website. It is not possible to get adsense account without any website/blog etc., as far as I know.

the same adsense code is being used for both nextra and mtnl users so I guess the one benefiting from this is adphonso else why mtnl would name it nextra
